New Patients:
Both the Patient Registration/Health History and HIPAA forms are required for ALL new patients. Please print out and complete these forms and bring them with you to your appointment.

Existing Patients:
If you are a previous patient, please use the Patient Registration Form to update your health history or address and bring it with you to your appointment.
Please be certain to also bring the following items to your appointment:
- A list of all your current medications and eye drops
- Current insurance information
- Health insurance cards
- Military ID
